Being discharged from hospital doesn't mean your parent is fully recovered. They may need help with mobility, medication, personal care or simply having someone there to make sure they're safe. Our hospital discharge care packages provide the support your parent needs during those crucial first days and weeks at home - reducing the risk of readmission and helping them get back on their feet.

Our hospital discharge care is tailored to your parent's specific recovery needs. This typically includes personal care support while mobility is limited, medication management for new or changed prescriptions, meal preparation and nutrition support, mobility assistance and fall prevention, companionship and emotional support during recovery, liaison with hospital teams, GPs and district nurses, and regular updates to keep you informed of progress. We can start care quickly - often within days of discharge - and adjust the level of support as your parent recovers.

For parents recovering from a stroke or major surgery, we provide reablement-focused care. This means encouraging independence wherever possible, supporting physiotherapy exercises, and gradually reducing care as confidence and ability return. The goal is always to help your parent regain as much independence as possible.

Hospital discharge care can be short-term (a few weeks of intensive support) or can transition into ongoing care if your parent's needs have changed permanently.
