Our six-step medication process
Every client. Every visit. No exceptions.
Assessment
Before care begins, we review all current medications, dosages, timing and any known interactions. We liaise with the GP and pharmacy to ensure we have an accurate and up-to-date medication list.
MAR chart set up
We create a Medication Administration Record (MAR chart) for every client. Every dose given or missed is recorded in real time on our Nourish care management system, with time-stamped notes.
Safe administration
Carers administer medication at the correct time, in the correct dose, using the correct method. We support tablets, liquids, patches, inhalers, eye drops and more. Controlled drugs are handled under strict protocols.
Monthly audit
Every month, our management team carries out a medication audit for every client. Stock levels are checked, MAR charts are reviewed, and any discrepancies are investigated and resolved immediately.
Prescription renewal
We monitor when prescriptions are due for renewal and liaise directly with GPs and pharmacies to ensure there are no gaps in supply. We collect prescriptions and coordinate delivery where needed.
QR emergency code
Every home we support has a QR code displayed prominently. If a paramedic attends, they can scan it to instantly access the care plan, current medications, allergies, GP details and DNAR status.
