Use this form to let us know about any visits you need to cancel. We'll confirm once your request has been actioned.

When will my cancellation be actioned?

This form is checked during office hours only. If the visit you are cancelling falls before the office reopens, please call our out-of-hours line instead: 01524 874110.

If the visit falls after the next office hours, your request will be actioned when the office reopens.

To cancel a single day, enter the same date in both fields. For a range, both dates are inclusive and care resumes the day after the "to" date.
