Use this form to request a shift swap with a colleague. Both carers must exchange shifts - this keeps your annual leave accruals protected.

If your shift swap is approved, it won't be recorded as an absence, so it won't impact your Bradford Score.

How it works

2.The office reviews the request and checks both shifts can be covered.
3.Once approved, both rotas are updated and you will receive confirmation.

Swap requests are reviewed during office hours. You will receive confirmation via email.
