Once submitted, please send the staff member an email containing:

  • -A thank you for attending their supervision and for their hard work
  • -A copy of the action points agreed above so they have them in writing
  • -A reminder of when their next supervision is due

This form will be sent to the office and saved to the staff member's supervision record.
